The rebate form, UPC bar code, and receipt must all pertain to the same product. Please save a copy of all the documentation you send in, to keep for your records.
Rebate cheque must be sent to a residence and not to a P.O. Box. There is a maximum of one (1) model per address. Samsung employees, agencies, resellers, and distributors are not eligible for a rebate.
Rebates can take a maximum of anywhere from 8-12 weeks to process and be sent. Due to the high volume of rebate claims received, we appreciate your patience while your rebate check is being processed.

Below is a copy of Samsung's UPC bar code from a monitor. Although the UPC bar codes may vary in design from product to product, each UPC barcode label should include the model name, model code, and product serial number. This is the label that MUST be submitted as proof of purchase.
